-credit card payment can be provided by phone or we can email a link for you to enter your payment details online
-Payment of $49.00 is collected
-Additional costs of $5/minute for extended visit with doctor will be added after a visit.
Step 2: Receive an ID# and link to our patient portal
-provide us with information about your sleep complaints, prior medical history, medications
-please have the dose and other medication information available (usually on medication bottles)
-provide us with the name of your primary care doctor and his/her contact information -this usually will take between 10 minutes to 30 minutes to complete.
Step 3: You will be redirected to our website where you will enter the chat to let us know that you are ready
-you will see your number in a queue and this will tell you the amount of time expected to wait.
Step 4: You will see the doctor and upon completion receive a management plan.
